Neutered dogs do not have functional testicles in the scrotum. During neutering, a veterinarian removes both testicles through a small incision in front of the scrotum. This procedure is also called orchiectomy. After surgery, the scrotum may look intact, but the testicles are gone.

After neutering, the scrotum often remains loose and swollen for a few days. This can make it look like the dog still has testicles. In some cases, a small skin flap or residual tissue is visible, but no actual testicles remain inside.
